Whatever ChibiUsa1995 said - but I'd also add this;
He left also for Sam's sake. His own condition was of course the main drive but if he staeyd, he would've only been in Sam's way. Sam was always torn between his love for Frodo; his beloved master who had by the time also become his best friend and brother - and his own dreams/family, especially now that he was married to Rosie and had a daughter. Sam loved Frodo (as a friend and master) so deeply and dedicatedly that there would've been no way for him to live happily enough if he'd always had to take care of both, Frodo, and his own family. Even if Frodo wasn't hurt and ill, Sam would've felt extremely responsible and dedicated to take care of him.

All Frodo's reasons are made very clear in the book's final chapter but they *are* also said in the film, in my opinion very clearly too in Frodo's lines as follows:

From the film:
"We were home. How do you pick up the threads of an old life? How do you go on... when in your heart you begin to understand... there is no going back? There are somethings that time cannot mend... some hurts that go too deep... that have taken hold."

"My dear Sam. You cannot always be torn in two. You will have to be one and whole for many years. You have so much to enjoy and to be and to do. Your part in the story will go on."

In short: Frodo left, because he couldn't take the horrible memories of the War of the Ring; all that evil that came from the ring that he had suffered from. And he would always feel the pain of the knife ( the morgul blade) that the Witch King stabbed him with. He left Middle Earth to the Grey Havens, because he didn't want to endure all that pain anymore; so he could start a new life.

My answer is based off of the Book, a little bit of the movie and pretty much any information on Frodo that I have found. :P

Hope this helps.

Frodo left because he felt like there was nothing left in the shire for him, and thinking that he went through that much, i wouldn't settle for the shire either.

Sam: It’s all wrong. By rights we shouldn’t even be here. But we are. It’s like in the great stories, Mr. Frodo. The ones that really mattered. Full of darkness and danger they are. And sometimes you didn’t want to know the end because how could the end be happy? How could the world go back to the way it was when so much bad had happened? But in the end, it’s only a passing thing, this shadow. Even darkness must pass. A new day will come. And when the sun shines it will shine out the clearer. Those were the stories that stayed with you. That meant something. Even if you were too small to understand why. But I think Mr. Frodo, I do understand. I know now. Folk in those stories had lost of chances of turning back only they didn’t. They kept going because they were holding on to something.

Frodo: What are we holding on to Sam?

Sam: That’s there’s some good in this world, Mr. Frodo. And it’s worth fighting for.

It was the highest honor for the elves to bring Frodo with them. It's clearer in the book what's going on. But they were going to the Grey Havens, which is filled with eternal peace. Frodo suffered greatly as the ring already had such a huge effect on him. As did his stab wound. So this was his way of finding peace.

If you can get a hold of the third book there is a good explanation in the Index under "Frodo" and also "Rings of Power" I don't remember it well enough at the moment and I don't want to mess up the answer. Also in the commentaries and special features in the DVDs they give a lot of information from the diaries and stories of Tolkien.

Well, Frodo left because after all he had been trough, he couldn't return to the life he had before the ring came to his hands...so much pain turned him seriously...and he had to go and let Sam, Merry and Pipin be happy...
