'smells like children' features many dirty words. moreover its 8th track "Fuck Frankie": offensive and explicit.

Oh, Smells Like Children is indeed a very extreme album, his 2nd most extreme I'd say. Especially if you get the bootleg version called "Resident Evil" (tracks can be found on Youtube). Manson himself said that album, before the record industry forced him to change it, was very disturbing to him. And its theme is all about preying on the innocent, a disturbing theme if I ever heard one. However, AntiChrist Superstar is in my opinion his most extreme album because he really went all out with it. If you read what he says about it in interviews that was a very depressing time in his life and he basically poured all of his pain and rage into making it, and he and Twiggy believe they had a telepathic link when they wrote the music together. Along with that and the message sent through the songs, inspired by Manson's reading of the works of Nietzsche, it is very extreme and shocking in that it seems to be his first truly organized attack against the traditional morals held dear by American society. Very powerful stuff. ^^

Smells Like Children is another one. This one is my favorite album though. I love "Portrait of An American Family" As well, which has "My Monkey" and "Cake and Sodomy." Which were pretty shocking and awesome.

Define extreme? This one was extreme in the sense of shock in many ways. He copared himself to god in this one, the cover makes it so it can't be sold at wallmart, NO ONE expected that after antichrist superstar, and to me it was just unexpected.

Lyric wise it might not have been that extreme, but I'm still going with Mechanical Animals

The first album I got, and it was pretty extreme, as I'm also Christian. And I was a bit surpriced there wasn't any certification of age on the cover or any warnings. Oh, and it was really weird with all those "empty" tracks. But I love the album!
