Alex hasn't come right out and said it to GA family but he does say it pretty much to one patient that his dad is dead and he told Izzie that he took care of him mom exactly the same way he took care of Ava/Rebecca when he was a child so she is either in a institute or died (but i think i recall him tell someone that his mom died when he was young)

They're not dead, last season Alex asked Izzie to go to Iwoa and meet his mum. As for his dad he told the patient that his dad used to hit his mum, so he beat him up, than his dad left and never came back, he never said he was dead...
