Technicallly she actually ends up with neither Spike nor Angel. In the second last episode of the seventh season of Buffy the Vampire Slayer, Angel makes a guest appearance and surprises Buffy. Although Buffy and Angel share a kiss she makes it clear she wants him to return to L.A. and that she was involved with Spike and has feelings for him. This is the episode where she gives her famous "Cookie Dough Speech" where she states that she believes she isn't ready for a long term relationship and may not be ready for quite some time. As Angel is leaving Buffy says that she sometimes thinks about them getting back together but if it could ever happen it could take years. Fast forward to the next episode "Chosen" (the final episode of the series) Buffy has chosen Spike as her champion to wear the magical amulet. As they are fighting the uber-vamps in the hellmouth, the amulet begins to work and Buffy is aware that Spike is not going to make it through alive. She encourages him to take it off and escape through the school with her but he says he wants to stay. She obviously doesn't leave him. The two share a powerful moment where they hold hands and their hands go on fire (watch the episode to really get the full picture!) and Buffy finally tells him she loves him. In the final season of the Buffy spinoff show Angel, Spike is resurrected (if thats what you can call it) from the amulet and joins Angel and the team at Wolfram and Hart. He doesn't tell Buffy he's alive because he wants her to have a normal life and remember him as a hero. Buffy makes no appearances in the final season of Angel so we never know who she would have officially chosen. Personally I like to believe it was Spike. She had fallen out of love with Angel long before, although she definelty still had feelings, but she really did love Spike when she realized that he had truly changed and risked his life to save the world.
