Ichigo cares a lot for Rukia. Whenever Rukia is in danger, Ichigo is the one who worry the most. If look at his facial expression, you will know there is something special between them. The way Ichigo treat Rukia is more than treating her as one of his best friend. That's just my opinion anyway.

Well, IMO Ichigo did really showcased very strong emotions towards Senna, You know, they were even teased for that. Though I'm not sure if it was love because you know Ichigo is overprotective almost over anyone (e.g. Nel, which he had just met), so it may've been just his personality.
Anyway, I don't consider her as Ichi's first love because, ultimately, she wasn't a cannon character so I feel it as betraying Kubo-sensei's only right to decide that fact.

This question makes no sense at all... He's only fifteen and wouldn't be having a relationship with a Shinigami, when he has presumably never even had a fling with a girl before... So, no-one.

@Zekrom676 At the beginning of the series, Ichigo was 15. During the series, he was 16. 17 months later after Aizen's defeat, Ichigo is 17. And this question threw me off lol. I'm just going to stick to Rukia for the heck of it...
