Who turned rose into a vampire and who turns Elena in to a vampire ?

I have no idea if Stefan turned Elena or if Damon turns elena

And when Elena and Matt go off the bridge, Elena has Damon's blood in her system. Meredith gave her Damon's blood to heal her injuries in between 3x21 and 3x22, but we don't see that.
So when Elena drowns, she has Damon's blood in her.
