It's from the beginning of Season 7 Episode 3 "The Girl Next Door." This episode was actually directed by Jensen. Interesting tidbit...the picture I added is Dean at the end of 7X2, "Hello Cruel World" as he's being transported to the ER. Must've been a tough ambulance ride! :)
