Couldn't Adam and Hoffman go away from their chains?

In the third movie Mathews was freed from the chains that held him in the bathroom by breaking his leg.I mean he got away once.Couldn't Adam (in Saw I and III)or Hoffman (in Saw 3D) go mad and do the same thing with their chains?

Adam yes because the toilet lid was broke he could have used that the same way eric Mathews did. Hoffman didn't have that chance because dr Gordon threw the saw out of the bathroom out of reach of Hoffman so he was left to die in the bathroom but there leaves one question is Hoffman alive or dead no one really knows.
