Sam wants Dean to see who he really is, he wants him to appreciate him and love him back the way he loves him. Wincest is definately implied, though it's nothing major. You could look past it if you wish.
Sam's job keeps him away from his wife and son more often that he'd like, when Sam forgets Jamie's birthday, he finally realizes he has to make a choice between his job and his family.
