Did Madoka Kaname die at the last episode (episode 12) of Madoka Magica?

Madoka Kaname is my favourite character in the series. I want to know why there are only 3 magical girls near the end of the episode (12)?
I feel sad if Madoka cannot transform from Goddess back to normal herself. If she can, I want to go out with her after school.

Thank you for your information Nric. Homura Akemi is the least favourite magical girl in Madoka Magica not the one I hate.

Oh, I get it. Madoka say 'Do your best' to Homura. I realized she becomes a goddess / Law of the Cycle.
