When the author asks what the reader thinks should happen next and When they're begging too much for reviews, or saying the story won't continue. I can tolerate the other ones.

Author notes inside the actual story are also irritating. They should be either at the start of a chapter or after a chapter has finished. It interrupts the flow otherwise and pulls the reader back out of the story.
