Hello... my question is if i should put in my fanfiction only Nadeshiko or i should put that nadeshiko really has a twin brother name Nagihiko????

You can really do whatever you want. It's a Fan Fiction right? I suggest that you use that Nadeshiko and Nagihiko are the same person, if you want to stick true to the story. But having them 2 separate people would make it interesting too. It's all up to you really.
